Your doctor must fill out the application detailing the nature of the temporary
condition. Please complete your portion.
Return the form to this office and a temporary placard
will be issued which
is good for
up to 90 days.
Permanent placards
are issued only from the IL Secretary of
State.

This is a service for Homer Township
Public Library District Residents who are temporarily
or permanently confined to their homes. Request
specific categories and/or titles of Books, Magazines
and Videos. Or our staff will be happy to choose the
selection. Even Library Membership can be obtained
through this service. Contact the Library for further
